About the Foundation
Founded in 2013 by Steve and his family, the Steve Smith Family Foundation was established as a way for the Smith Family to serve their community.

Charlotte Community Health Clinic (CCHC) Express Care offers same-day services for patients that are looking for immediate access to primary care services. The Clinic accepts Medicaid, Medicare, private insurance, as well as those without insurance.
